The greatest disease in the West today is not TB or leprosy; it is being unwanted, unloved, and uncared for. We can cure physical diseases with medicine, but the only cure for loneliness, despair, and hopelessness is love. There are many in the world who are dying for a piece of bread but there are many more dying for a little love. The poverty in the West is a different kind of poverty -- it is not only a poverty of loneliness but also of spirituality. There's a hunger for love, as there is a hunger for God.
It is astounding to me, and achingly sad, that with eighty thousand people on the waiting list for donated hearts and livers and kidneys, with sixteen a day dying there on that list, that more then half of the people in the position H's family was in will say no, will choose to burn those organs or let them rot. We abide the surgeon's scalpel to save our own lives, out loved ones' lives, but not to save a stranger's life. H has no heart, but heartless is the last thing you'd call her.
The night before brain surgery, I thought about death. I searched out my larger values, and I asked myself, if I was going to die, did I want to do it fighting and clawing or in peaceful surrender? What sort of character did I hope to show? Was I content with myself and what I had done with my life so far? I decided that I was essentially a good person, although I could have been better--but at the same time I understood that the cancer didn't care.I asked myself what I believed. I had never prayed a lot. I hoped hard, I wished hard, but I didn't pray. I had developed a certain distrust of organized religion growing up, but I felt I had the capacity to be a spiritual person, and to hold some fervent beliefs. Quite simply, I believed I had a responsibility to be a good person, and that meant fair, honest, hardworking, and honorable. If I did that, if I was good to my family, true to my friends, if I gave back to my community or to some cause, if I wasn't a liar, a cheat, or a thief, then I believed that should be enough. At the end of the day, if there was indeed some Body or presence standing there to judge me, I hoped I would be judged on whether I had lived a true life, not on whether I believed in a certain book, or whether I'd been baptized. If there was indeed a God at the end of my days, I hoped he didn't say, 'But you were never a Christian, so you're going the other way from heaven.' If so, I was going to reply, 'You know what? You're right. Fine.'I believed, too, in the doctors and the medicine and the surgeries--I believed in that. I believed in them. A person like Dr. Einhorn [his oncologist], that's someone to believe in, I thought, a person with the mind to develop an experimental treatment 20 years ago that now could save my life. I believed in the hard currency of his intelligence and his research.Beyond that, I had no idea where to draw the line between spiritual belief and science. But I knew this much: I believed in belief, for its own shining sake. To believe in the face of utter hopelessness, every article of evidence to the contrary, to ignore apparent catastrophe--what other choice was there? We do it every day, I realized. Dispiritedness and disappointment, these were the real perils of life, not some sudden illness or cataclysmic millennium doomsday. I knew now why people fear cancer: because it is a slow and inevitable death, it is the very definition of cynicism and loss of spirit.So, I believed.

The disease of the soul is both more common and more deadly than the disease of the body. Just as medicine is the art devoted to healing the body, so philosophy is the art devoted to healing the soul, curing it of improper emotions, false beliefs, and faulty judgments, which are the causes of so much hardship and handicap. To heal the body one turns to the practitioner of the art of healing the body, but to heal the soul there is no doctor to turn to, and each of us is left to become that doctor unto himself. Yet, this need not stop us from exhorting others to imitate us in the godly art, in the forlorn hope that they might transform themselves into better citizens for Athens and better companions for us.
If your leg is in a cast, it's really dumb to sit in front of your computer doing unnecessary stuff with it hanging down. Your leg will swell and heal slower, if at all. The talk of sin is of course to many a big turn-off; to others, an even bigger myth - because in reality, sin is like the spiritual equivalent of a microscopic parasite, or a virus, or better yet even, an infectious disease. And just as one might never know of, until visiting a competent doctor, the tiny pathogens progressively eroding one's body, so we might never know that in sin we are eroding our being and losing direction until hearing the Word of God rightfully applied. Therefore I ask, which of the doctors would then be the more competent: the one who finds the problem and gives the solution, or the one who willfully ignores the problem (or rather finds the problem when it is much too late)? Seldom does anyone write off the knowledge of medicine for the physical body as primitive practice, so neither must the knowledge of the Word of God for one's spiritual well-being remain written off as primitive practice - quite the opposite really. As it is written thus: 'Lean not on your own understanding.

Another explanation for the failure of logic and observation alone to advance medicine is that unlike, say, physics, which uses a form of logic - mathematics - as its natural language, biology does not lend itself to logic. Leo Szilard, a prominent physicist, made this point when he complained that after switching from physics to biology he never had a peaceful bath again. As a physicist he would soak in the warmth of a bathtub and contemplate a problem, turn it in his mind, reason his way through it. But once he became a biologist, he constantly had to climb out of the bathtub to look up a fact.

Eradication represents a complete change of philosophy and a recognition of the equal rights of all citizens to protection from infection, no matter where they live. Having a body, we have seen, does not entail knowing a body. Whereas a cow automatically eats whatever grasses supply needed nutrients, people must determine for themselves what to put into their bodies, with the result that there is room to make mistakes. Mistakes arise, in part, from ignorance. Yet ignorance is not the only problem produced by this arrangement. The fact that we are not compelled by our bodies' precise needs—understood as particular kinds of food and drink, rather than food and drink tout court—allows the formation of desires that have little or nothing to do with the needs on which bodily health depends.
Xenophon tells us that Socrates never neglected the body and did not praise those who did. We can imagine that it was because the physical body—volatile, unseen, and implicated in an automatized natural world—could seem so daemonic that entrusting life, both biological life and ethical life, to its dynamics could seem like ceding control of the human.
I emphasize this because some of my colleagues, for whose academic attainments I have great respect, argue" 'You assume too much; this is not proved; this is not strictly scientific. We disagree with your neurology and your psychiatry is misleading, therefore you must be wrong.' My reply has been, with all humility: 'Yes, of course,' and I have returned to the labor ward to be greeted by happy women with their newborn babies in their arms: 'How right you are, Doctor, it is so much easier that way.' That is what really matters to the clinician. He should use the method that gives the best and safest result from all points of view until something better is discovered.
Emblematic of this era was the prolific Viennese surgeon Theodor Billroth. Born in 1821, Billroth studied music and surgery with almost equal verve. (The professions still often go hand in hand. Both push manual skill to its limit; both mature with practice and age; both depend on immediacy, precision, and opposable thumbs.)
We often use the Bible as a source for personal validation and defense, a sidekick and a shield, but these will prove ineffective without first the other part. We must also allow ourselves to be wounded by it. We tend to forget its authority - that it is a double-edged sword. Our decrepit, depraved hearts must be completely ripped out in order to welcome that of God.
We are often given pills or fluids to help remedy illness, yet little has been taught to us about the power of smell to do the exact same thing. It is known that the scent of fresh rosemary increases memory, but this cure for memory loss is not divulged by doctors to help the elderly. I also know that the most effective use of the blue lotus flower is not from its dilution with wine or tea – but from its scent. To really maximize the positive effects of the blue lily (or the pink lotus), it must be sniffed within minutes of plucking. This is why it is frequently shown being sniffed by my ancient ancestors on the walls of temples and on papyrus. Even countries across the Orient share the same imagery. The sacred lotus not only creates a relaxing sensation of euphoria, and increases vibrations of the heart, but also triggers genetic memory - and good memory with an awakened heart ushers wisdom.
In the early 1970s, racial and gender discrimination was still prevalent. The easy camaraderie prevailing in the operating room evaporated at the completion of surgical procedures. There was an unspoken pecking order of seating arrangements at lunch among my fellow physicians. At the top were the white male 'primary producers' in prestigious surgical specialties. They were followed by the internists. Next came the general practitioners. Last on the list were the hospital-based physicians: the radiologists, pathologists and anaesthesiologists - especially non-white, female ones like me. Apart from colour, we were shunned because we did not bring in patients ourselves but, like vultures, lived off the patients generated by other doctors. We were also resented because being hospital-based and not having to rent office space or hire nursing staff, we had low overheads. Since a physician's number of admissions to the hospital and referral pattern determined the degree of attention and regard accorded by colleagues, it was safe for our peers to ignore us and target those in position to send over income-producing referrals. This attitude was mirrored from the board of directors all the way down to the orderlies.
Observing the medical histories of various neurological syndromes is like observing the fascinating nerve cells of the human brain in action, while they construct what we so proudly call the Human Consciousness. They remind us of the overwhelming aspects of human silliness. They remind us how such a simple natural response of the human Biology, is misinterpreted as the “last surviving mystery” of this planet.

It is possible to hold one's head up after an error, to admit that errors are part and parcel of human existence, even in medicine. It is possible to see the error as an aspect of oneself, not the defining characteristic of oneself.

With few exceptions, the immune system does not appear to recognize cancers within an individual as foreign, because they are actually part of the self.
Eventually it became clear that our emotions, attitudes, and thoughts profoundly affect our bodies, sometimes to the degree of life or death. Soon mind-body effects were recognized to have positive as well as negative impacts on the body. This realization came largely from research on the placebo effect—the beneficial results of suggestion, expectation, and positive thinking.
Let us be the ones who say we do not accept that a child dies every three seconds simply because he does not have the drugs you and I have. Let us be the ones to say we are not satisfied that your place of birth determines your right for life. Let us be outraged, let us be loud, let us be bold.

Under the guise of helping the sick, oppressed and hopeless, psychiatry is paving the way for authoritarian governments to suppress a whole society furthermore, with drugs and obscene practices that promote, not only hypnotic suggestions, but also highly suggestible individuals which, otherwise, would oppose a whole repressive system that threatens both their existence and the existence of future generations on Earth. And so, one can very well say that, psychiatry, aided by pharmaceutical corporations and power-driven governments, or merely governments fearful of their own people and the extinction of immoral politics, will contribute vastly to the extermination and full extinction of the human race.
My sadness is beautiful. It infuses everything I do. It is at the core of my identity and always has been, just as happiness is in some people. I refuse to be told that it's a flaw. I will not mute it with medications for the sake of society. I will hold it close to me and celebrate it rightfully while the rest of the world fails to see it for what it is and it will be their loss.
As Mollie said to Dailey in the 1890s: "I am told that there are five other Mollie Fanchers, who together, make the whole of the one Mollie Fancher, known to the world; who they are and what they are I cannot tell or explain, I can only conjecture." Dailey described five distinct Mollies, each with a different name, each of whom he met (as did Aunt Susan and a family friend, George Sargent). According to Susan Crosby, the first additional personality appeared some three years after the after the nine-year trance, or around 1878. The dominant Mollie, the one who functioned most of the time and was known to everyone as Mollie Fancher, was designated Sunbeam (the names were devised by Sargent, as he met each of the personalities). The four other personalities came out only at night, after eleven, when Mollie would have her usual spasm and trance. The first to appear was always Idol, who shared Sunbeam's memories of childhood and adolescence but had no memory of the horsecar accident. Idol was very jealous of Sunbeam's accomplishments, and would sometimes unravel her embroidery or hide her work. Idol and Sunbeam wrote with different handwriting, and at times penned letters to each other.The next personality Sargent named Rosebud: "It was the sweetest little child's face," he described, "the voice and accent that of a little child." Rosebud said she was seven years old, and had Mollie's memories of early childhood: her first teacher's name, the streets on which she had lived, children's songs. She wrote with a child's handwriting, upper- and lowercase letters mixed. When Dailey questioned Rosebud about her mother, she answered that she was sick and had gone away, and that she did not know when she would be coming back. As to where she lived, she answered "Fulton Street," where the Fanchers had lived before moving to Gates Avenue.Pearl, the fourth personality, was evidently in her late teens. Sargent described her as very spiritual, sweet in expression, cultured and agreeable: "She remembers Professor West [principal of Brooklyn Heights Seminary], and her school days and friends up to about the sixteenth year in the life of Mollie Fancher. She pronounces her words with an accent peculiar to young ladies of about 1865." Ruby, the last Mollie, was vivacious, humorous, bright, witty. "She does everything with a dash," said Sargent. "What mystifies me about 'Ruby,' and distinguishes her from the others, is that she does not, in her conversations with me, go much into the life of Mollie Fancher. She has the air of knowing a good deal more than she tells.
The physical shape of Mollies paralyses and contortions fit the pattern of late-nineteenth-century hysteria as well — in particular the phases of "grand hysteria" described by Jean-Martin Charcot, a French physician who became world-famous in the 1870s and 1880s for his studies of hysterics...""The hooplike spasm Mollie experienced sounds uncannily like what Charcot considered the ultimate grand movement, the arc de de cercle (also called arc-en-ciel), in which the patient arched her back, balancing on her heels and the top of her head...""One of his star patients, known to her audiences only as Louise, was a specialist in the arc de cercle — and had a background and hysterical manifestations quite similar to Mollie's. A small-town girl who made her way to Paris in her teens, Louise had had a disrupted childhood, replete with abandonment and sexual abuse.She entered Salpetriere in 1875, where while under Charcot's care she experienced partial paralysis and complete loss of sensation over the right side of her body, as well as a decrease in hearing, smell, taste, and vision. She had frequent violent, dramatic hysterical fits, alternating with hallucinations and trancelike phases during which she would "see" her mother and other people she knew standing before her (this symptom would manifest itself in Mollie). Although critics, at the time and since, have decried the sometime circus atmosphere of Charcot's lectures, and claimed that he, inadvertently or not, trained his patients how to be hysterical, he remains a key figure in understanding nineteenth-century hysteria.
Rather than being medicalized or romanticized, mental disorders, or mental dis-eases, should be understood as nothing less or more than what they are, an expression of our deepest human nature. By recognizing their traits in ourselves and reflecting upon them, we may be able both to contain them and to put them to good use. This is, no doubt, the highest form of genius.

There is a moral imperative to seeing mental health through the same lens we use for other pathologies or illnesses. Being sad or overwhelmed is normal, much as being short of breath after a run is normal. Both become abnormal when they happen with no apparent cause and are hard to stop. Those situations need medical attention.

How the sadness is handled by the physician has a powerful impact on the medical care received by the patients. If the grief is relentlessly suppressed--as in Eva's experience during residency--the result can be a numb physician who is unable to invest in a new patient. This lack of investment can lead to rote medical care--impersonal at best, shoddy at worst. At the other end of the spectrum is the doctor who is inundated with grief and can't function because of the overwhelming sorrow. Burnout is significant in both these cases, and that erodes the quality of medical care.

What if we as animal lovers could change our focus from individual symptoms to an awareness of the whole animal and their innate ability to heal? Radionics was conceived as a diagnostic and treatment technology at a time when modern electronic theory and biomedicine had not become the dominant sciences they are today. Early radionic devices incorporated the new discoveries of radio and electronics into their design. During that period, the functional assumptions of radionic technology did not seem as implausible as it does today. However, it wasn't long before radionics became outmoded and completely non-scientific. As Mizrach has noted, radionics continued to appropriate the methods of orthodox science into its design and terminology, making the probability of understanding what it could accomplish even more difficult to assess. I will examine this appropriation in a spirit of tolerance, given the state of electronics and medicine circa 1910, when radionics was first discovered. I will do so in order to shift the focus of this interesting technology from the scientific to the metaphysical, where the reader not limited by a need for scientific approval can evaluate it. My aim is to provide a reasonable means of evaluating radionic technology as an artistic methodology.

The surgical resident interested in learning trauma will bypass a residency at a quiet community hospital for a residency at a fast-paced Level 1 trauma center treating a high volume of trauma patients. A Level 1 trauma center residency is far more rigorous—and not particularly glamorous—but the intensive culture of a dedicated trauma center will cultivate the decisive judgment and action required of a surgeon specializing in trauma. But such is the nature of man that as soon as you begin to force him to do a thing, from that moment he begins to seek ways by which he can avoid doing the thing you are trying to force upon him. A man with malaria parasites in his blood is a danger to his companions. To kill all the parasites, he was then required to continue doses of quinine a week or ten days after his fever. When the convalescing men were given their daily dose of quinine they would manage to throw their tablets out of the dispensary window. The old turkey-gobbler pet of the hospital gobbled up all the tablets he could find. He became so dissipated he finally developed a species of blindness caused by too much quinine. I cannot vouch for this, but I was often twitted with this story as an illustration of how the men were treating prophylactic quinine.
I am a cuddly atheist... I am against creationism being taught in schools because there is empirical evidence that it is a silly notion... I am passionately concerned about the rise in pseudo-science; in beliefs in alternative medicine; in creationism. The idea that somehow it is based on logic, on rational arguments, but it's not. It doesn't stand up to empirical evidence.In the same way in medicine, alternative medicines like homeopathy or new age therapies – reiki healing – a lot of people buy into it and it grates against my rationalist view of the world. Seeing modern health care from the other side, I can say that it is clearly not set up for the patient. It is frequently a poor arrangement for doctors as well, but that does not mitigate how little the system accounts for the patient's best interest. McKusick's belief in this paradigm-the focus on disability rather than abnormalcy-was actualized in the treatment of patients in his clinic. Patients with dwarfism, for instance, were treated by an interdisciplinary team of genetic counselors, neurologists, orthopedic surgeons, nurses, and psychiatrists trained to focus on specific disabilities of persons with short stature. Surgical interventions were reserved to correct specific deformities as they arose. The goal was not to restore "normalcy"-but vitality, joy, and function. McKusic had rediscovered the founding principles of modern genetics in the realm of human pathology. In humans as in wild flies, genetic variations abounded. Here too genetic variants, environments, and gene-environment interactions ultimately collaborated to cause phenotypes-except in this case, the "phenotype" in question was disease. Here too some genes had partial penetrance and widely variable expressivity. One gene could cause many diseases, and one disease could be caused by many genes. And here too "fitness" could not be judged in absolutes. Rather the lack of fitness-illness [italicized, sic] in colloquial terms- was defined by the relative mismatch between an organism and environment.

Unlike the more "modern" notions, in shamanic society health is not the absence of feeling; no more so is it the absence of pain. Health is seeking out all of the experiences of Creation and turning them over and over, feeling their texture and multiple meanings. Health is expanding beyond one's singular state of consciousness to experience the ripples and waves of the universe.
Arguably the greatest technological triumph of the century has been the public-health system, which is sophisticated preventive and investigative medicine organized around mostly low- and medium-tech equipment; ... fully half of us are alive today because of the improvements.
The conditions necessary for devastating epidemics or pandemics just didn't exist until the agricultural revolution. The claim that modern medicine and sanitation save us from infectious diseases that ravaged pre-agricultural people (something we hear often) is like arguing that seat belts and air bags protect us from car crashes that were fatal to our prehistoric ancestors.
Until fairly recently, every family had a cornucopia of favorite home remedies--plants and household items that could be prepared to treat minor medical emergencies, or to prevent a common ailment becoming something much more serious. Most households had someone with a little understanding of home cures, and when knowledge fell short, or more serious illness took hold, the family physician or village healer would be called in for a consultation, and a treatment would be agreed upon. In those days we took personal responsibility for our health--we took steps to prevent illness and were more aware of our bodies and of changes in them. And when illness struck, we frequently had the personal means to remedy it. More often than not, the treatment could be found in the garden or the larder. In the middle of the twentieth century we began to change our outlook. The advent of modern medicine, together with its many miracles, also led to a much greater dependency on our physicians and to an increasingly stretched healthcare system. The growth of the pharmaceutical industry has meant that there are indeed "cures" for most symptoms, and we have become accustomed to putting our health in the hands of someone else, and to purchasing products that make us feel good. Somewhere along the line we began to believe that technology was in some way superior to what was natural, and so we willingly gave up control of even minor health problems.
The U.S. has a so-called health care system that has nothing to do with the promotion of health. Those who run this system do not care about your health, and it's far from being a system. It's a fragmented patchwork of procedure-oriented services that are meshed in a voluminous trail of paper payments, with little relevance to community-based needs. This misdirected, disease-managed non-care system of symptom suppression demands more and more treatment at higher and higher costs. If they cared at all, you'd be treated like a human, not like a number resembling, quite frankly, the ear tags on a cattle herd.

I have blogged previously about the dangerous and deadly effects of science denialism, from the innocent babies unnecessarily exposed to deadly diseases by other kids whose parents are anti-vaxxers, to the frequent examples of how acceptance of evolution helps us stop diseases and pests (and in the case of Baby Fae, rejection of evolution was fatal), to the long-term effects of climate denial to the future of the planet we all depend upon. But one of the strangest forms of denialism is the weird coalition of people who refuse to accept the medical fact that the HIV virus causes AIDS. What the heck? Didn’t we resolve this issue in the 1980s when the AIDS condition first became epidemic and the HIV virus was discovered and linked to AIDS? Yes, we did—but for people who want to deny scientific reality, it doesn’t matter how many studies have been done, or how strong the scientific consensus is. There are a significant number of people out there (especially among countries and communities with high rates of AIDS infections) that refuse to accept medical reality. I described all of these at greater length in my new book Reality Check: How Science Deniers Threaten our Future.
Why has the medical profession not taken advantage of the help available from evolutionary biology, a well-developed branch of science with great potential for providing medical insights? One reason is surely the pervasive neglect of this branch of science at all educational levels. Religious and other sorts of opposition have minimized the impact in general education of Darwin's contributions to our understanding of ourselves and the world we live in.
Long before there were effective treatments, physicians dispensed prognoses, hope, and, above all, meaning. When something terrible happens-and serious disease is always terrible-people want to know why. In a pantheistic world, the explanation was simple-one god had caused the problem, another could cure it. In the time since people have been trying to get along with only one God, explaining disease and evil has become more difficult. Generations of theologians have wrestled with the problem of theodicy-how can a good God allow such bad things to happen to good people? Darwinian medicine can't offer a substitute for such explanations. It can't provide a universe in which events are part of a divine plan, much less one in which individual illness reflects individual sins. It can only show us why we are the way we are, why we are vulnerable to certain diseases. A Darwinian view of medicine simultaneously makes disease less and more meaningful. Diseases do not result from random or malevolent forces, they arise ultimately from past natural selection. Paradoxically, the same capacities that make us vulnerable to disease often confer benefits. The capacity for suffering is a useful defense. Autoimmune disease is a price of our remarkable ability to attack invaders. Cancer is the price of tissues that can repair themselves. Menopause may protect the interests of our genes in existing children. Even senescence and death are not random, but compromises struck by natural selection as it inexorably shaped out bodies to maximize the transmission of our genes. In such paradoxical benefits, some may find a gentle satisfaction, even a bit of meaning-at least the sort of meaning Dobzhansky recognized. After all, nothing in medicine makes sense except in the light of evolution.
People underestimate the importance of dilligence as a virtue. No doubt it has something to do with how supremely mundane it seems. It is defined as "the constant and earnest effort to accomplish what is undertaken."... Understood, however, as the prerequisite of great accomplishment, diligence stands as one of the most difficult challenges facing any group of people who take on tasks of risk and consequence. It sets a high, seemingly impossible, expectation for performance and human behavior.
We live in a global village, Neel, where billions of voices babble simultaneously, and in this village a new hierarchy is being established, a new caste-system is being created. Only this time, it is money that sets the tone. Whoever has the most money buys the biggest loudspeaker and is the neo-Brahmin of this new world order. If the ninety-year-old neo-Brahmin on the other side of the earth is terrified of antibiotic resistant flesh-eating bacteria, we must think twice before offering treatment to a twenty-four-year old here. These are the new rules of our global village.
I am rather disturbed by the fact that so many people—who are neither medical professionals nor trans themselves—would want to hear all of the gory details regarding transsexual physical transformations, or would feel that they have any right to ask us about the state of our genitals.
One of the most interesting histories of what comes of rejecting science we may see in Islam, which in the beginning received, accepted, and even developed the classical legacy. For some five or six rich centuries there is an impressive Islamic record of scientific thought, experiment, and research, particularly in medicine. But then, alas! the authority of the general community, the Sunna, the consensus—which Mohammed the Prophet had declared would always be right—cracked down. The Word of God in the Koran was the only source and vehicle of truth. Scientific thought led to 'loss of belief in the origin of the world and in the Creator.' People who foster dependence on illicit drugs such as heroin are regarded among the most unscrupulous pariahs of modern civilisation. In contrast, pushers of licit drugs tend to be viewed as altruistically motivated purveyors of social good.
There are many arts and sciences of which a miner should not be ignorant. First there is Philosophy, that he may discern the origin, cause, and nature of subterranean things; for then he will be able to dig out the veins easily and advantageously, and to obtain more abundant results from his mining. Secondly there is Medicine, that he may be able to look after his diggers and other workman ... Thirdly follows astronomy, that he may know the divisions of the heavens and from them judge the directions of the veins. Fourthly, there is the science of Surveying that he may be able to estimate how deep a shaft should be sunk ... Fifthly, his knowledge of Arithmetical Science should be such that he may calculate the cost to be incurred in the machinery and the working of the mine. Sixthly, his learning must comprise Architecture, that he himself may construct the various machines and timber work required underground ... Next, he must have knowledge of Drawing, that he can draw plans of his machinery. Lastly, there is the Law, especially that dealing with metals, that he may claim his own rights, that he may undertake the duty of giving others his opinion on legal matters, that he may not take another man's property and so make trouble for himself, and that he may fulfil his obligations to others according to the law.
I had seen that once before, bleeding water. A little baby I worked on as a resident in training. That poor kid had been shot as well—his father had blasted away the top of his head with a shotgun—and we couldn't begin to stop the bloodletting in that case. "Looking pretty thin down here," I hollered when the stuff coming out his wounds was no more than pink salt water. That baby's heart stopped, started, stopped and started a dozen times before it finally gave up the ghost and we pronounced him. I could have read a newspaper through the watery stuff coming out his veins by then.
Of the many 'firsts' with which I have been involved at the Texas Heart Institute —including the first successful human heart transplant in the United States and the first total artificial heart transplant in the world—the achievement that may have the greatest impact on health care did not occur in the operating room or in the research laboratory. Unlike ‘mere’ medical or physical disorders, mental disorders are not just problems. If successfully navigated, they can also present opportunities. Simply acknowledging this can empower people to heal themselves and, much more than that, to grow from their experiences.
Other pressing problems with the current medical model [of mental disorder] is that it encourages false epidemics, most glaringly in bipolar disorder and ADHD, and the wholesale exportation of Western mental disorders and Western accounts of mental disorder. Taken together, this is leading to a pandemic of Western disease categories and treatments, while undermining the variety and richness of the human experience.
So, if you're a doctor, how can you recognize that you're having a feeling? Some tips from Dr. Zinn:Most emotions have physical counterparts. Anxiety may be associated with a tightness of the abdomen or excessive diaphoresis; anger may be manifested by a generalized muscle tightness or a clenching of the jaw; sexual arousal may be noted by a tingling of the loins or piloerection; and sadness may be felt by conjunctival injection or heaviness of the chest.
Love is a chemical reaction, but it cannot be fully understood or defined by science. And though a body cannot exist without a soul, it too cannot be fully understood or defined by science. Love is the most powerful form of energy, but science cannot decipher its elements. Yet the best cure for a sick soul is love, but even the most advanced physician cannot prescribe it as medicine.
I submit a body of facts which cannot be invalidated. My opinions may be doubted, denied, or approved, according as they conflict or agree with the opinions of each individual who may read them; but their worth will be best determined by the foundation on which they rest—the incontrovertible facts.
Empathy requires being attuned to the patient's perspective and understanding how the illness is woven into this particular persons' life. Last--and this is where doctors often stumble--empathy requires being able to communicate all of this to the patient.
It can be rough going to maintain both composure and empathy in these situations, but a doctor's failure to do that is probably the number one reason why patients feel dissatisfied with their physicians and end up doctor-shopping endlessly.
But while the patient does bear some responsibility, I believe that the onus falls more heavily on the doctor to be attuned to the factors--cultural, ethnic, or just personal style--that influence how patients present their symptoms.
What these older physicians exhibited is termed clinical curiosity. They stroke to understand their patients in order to elucidate the underlying medical conditions. This thoroughness, patience, and dogged curiosity may have been ingrained in them because they trained at a time when they were no rapid CTs or MRIs. But even now, when these diagnostic tools are at their fingertips, these physicians maintain this approach to patients, one that serves to appreciate the dignity and uniqueness of each patient and his or her illness.
In the middle of the cemetery is a grassy plane, strangely vacant. There are no granite tombs or crumbling concrete, just a sun-washed treeless patch of green known as "No Man's Land." Here 1,500 unidentified bodies are buried. At one time, their skin burned with yellow fever; now they lie in a cool, dark place where long ago their arms and legs, hands and feet, were intertwined for eternity.
We live in a drug culture! Drugs are everywhere and touted as the panacea for every ailment in our society. We have drugs for hyper children, drugs for depression—some of the most insidious drugs ever—, drugs for allergies, drugs for acne, drugs for emphysema and drugs for erectile disfunction—maybe the most useful of them all. And let’s not forget the side effects of these wonder drugs! It’s cliche to even talk about drug advertisements and the laundry list of side effects tacked onto the end of them, usually rattled off at warp speed by someone on loan from the local auction house. I’ve seen ads for acne medicines that include side effects that are potentially fatal! Seriously? “Hey! Buy our Acne-Magic Drug! You’ll have crystal clear skin! In your coffin!” What the hell is wrong with us?
Generally places that are comfortable with excellence don’t call themselves centers of excellence. Has anyone heard of a Princeton University Center of Excellence? Memorial Sloan-Kettering Cancer Center of Excellence?
Critical thinking helps us to more clearly understand situations, patients, colleagues, and our agendas, negative emotions, attitudes, motivations, talents, and growing edges. This not only helps us to have a greater grasp of reality but also stops the drain of psychological energy that is necessary to be defensive or to protect our image. Because critical thinking is not natural, although we may think it is for us, it takes discipline, a willingness to face the unpleasant, and a stamina that allows one not to become unduly frustrated when we do not achieve results as quickly as we prefer with respect to our insights and growth.
The health benefits, both mental and physical, of humor are well documented. A good laugh can diffuse tension, relieve stress, and release endorphins into your system, which act as a natural mood elevator. In Norman Cousin's book, Anatomy of an Illness, Cousin's describes the regimen he followed to overcome a serious debilitating disease he was suffering from. It included large doses of laughter and humor. Published in 1976, his book has been widely accepted by the medical community.
If my efforts have led to greater success than usual, this is due, I believe, to the fact that during my wanderings in the field of medicine, I have strayed onto paths where the gold was still lying by the wayside. It takes a little luck to be able to distinguish gold from dross, but that is all.
We look for medicine to be an orderly field of knowledge and procedure. But it is not. It is an imperfect science, an enterprise of constantly changing knowledge, uncertain information, fallible individuals, and at the same time lives on the line. There is science in what we do, yes, but also habit, intuition, and sometimes plain old guessing. The gap between what we know and what we aim for persists. And this gap complicates everything we do.

The question then, is not whether one accepts the responsibility. Just by doing this work, one has. The question is, having accepted the responsibility, how one does such work well.

Who was I, vowing calmly among all these necktied young men to steal life out of nature's jaws, every old time we got half a chance and a paycheck?... I see people who have made every bad move anyone could make, made every major mistake there was to be made, and by the time I see them, they are paying for it, sometimes with their lives. The freedom of patient speech is necessary if the doctor is to get clues about the medical enigma before him. If the patient is inhibited, or cut off prematurely, or constrained into one path of discussion, then the doctor may not be told something vital. Observers have noted that, on average, physicians interrupt patients within eighteen seconds of when they begin telling their story.

Here in Bosnia I had already seen several cases of rheumatic fever and a case we thought was miliary tuberculosis, diseases now rare in America. It was sobering to think that the mundane process of vaccinating these children might ultimately save more lives than any UN-brokered peace treaty.
Primum non nocere, 'First, don't make things worse,' was an essential principle of Hippocrates' medicine. Nowadays, unfortunately, it seems to have been forgotten. Conventional modern medicine aims at getting rid of patients' symptoms. Little, if any consideration is given to the fact that some of these symptoms may actually be used by the body in an attempt to correct deeper disorders. When this is the case, suppressing the symptom does not necessarily help the patient.
In the nineteenth century, girls who learned to develop orgasmic capacity by masturbation were regarded as medical problems. Often they were 'treated' or 'corrected' by amputation or cautery of the clitoris or 'miniature chastity belts,' sewing the vaginal lips together to put the clitoris out of reach, and even castration by surgical removal of the ovaries. But there are no references in the medical literature to the surgical removal of testicles or amputation of the penis to stop masturbation in boys.In the United States, the last recorded clitoridectomy for curing masturbation was performed in 1948-- on a five-year-old girl.

Words began to feel as weightless as the breath that carried. Stepping back, I realized that I was merely confirming what I already knew: I wanted that direct experience. It was only in practicing medicine that I could pursue a serious biological philosophy. Moral speculation was puny compared to moral action.
I should never have been happy in any profession that did not call forth the highest intellectual strain, and yet keep me in good warm contact with my neighbors. There is nothing like the medical profession for that: one can have the exclusive scientific life that touches the distance and befriend the old fogie in the parish too.
But against sandfly fever one could be inoculated, and I have another, hideously vivid picture of a great menacing brute of a doctor sticking a Thing that ended in a vicious needle into my mother's arm. Afterwards, everybody said how wonderful the doctor had been, because he continued calmly giving the inoculation while I was prised off him, instead of breaking the needle in my mother's arm. But nobody said how brave it was of me, only three years old, when all is said and done, and gone in the legs at that, to take on such fearful odds for the sake of love.
In the days of Columbus most medical practices were as much superstition as science. Because of infections, operations were not often performed, except for amputations under dire battlefield conditions. Most of the time these attempts to rectify an abnormality ended in disaster. Now things are different, with positive results being expected and are so frequent that people depend on elective surgery to enhance their lives.
When our time is limited and we are uncertain about how best to serve our priorities, we are forced to deal with the fact that both the experiencing self and the remembering self matter. We do not want to endure long pain and short pleasure. Yet certain pleasures can make enduring suffering worthwhile. The peaks are important, and so is the ending.
In the pre-war era when itinerant home-remedy salesmen still wandered the country, they had a traditional patter for selling a potion that was supposed to be particularly effective in treating burns and cuts. A toad with four legs in front and six behind would be placed in a box with mirrors lining the four walls. The toad, amazed at its own appearance from every angle, would break into an oily sweat. This sweat would be collected and simmered for 3,721 days while being stirred with a willow branch. The result was the marvelous potion.When writing about myself, I feel something like that toad in the box.
I was temperamentally better suited to a cognitive discipline, to an introspective field—internal medicine, or perhaps psychiatry. The sight of the operating theater made me sweat. The idea of holding a scalpel caused coils to form in my belly. (It still does.) Surgery was the most difficult thing I could imagine.And so I became a surgeon.
A shoemaker, in making a pair of shoes, cannot spoil a scrap of leather without having to bear the loss; but in our business we may spoil a man without its costing us a farthing. The blunders are never put down to us, and it is always the fault of the fellow who dies. The best of this profession is, that there is the greatest honesty and discretion among the dead; for you never find them complain of the physician who has killed them.
If I’ve learned anything, it's that we know next to nothing. Disease is a mystery. Health is inscrutable. The body itself is scarcely understood; we can only examine the secrets of the dead. And in all that dark ignorance, we're sometimes granted a rare moment of illumination. The truth is a gift.
...my patient needed a great deal of reassurance that there had been nothing unusual about the way her mother died, that she had not done something wrong to prevent her mother from experiencing that "spiritual" death with dignity that she had anticipated. All of her efforts and expectations had been in vain, and now this very intelligent woman was in despair. I tried to make clear to her that the belief in the probability of death with dignity is our, and society's, attempt to deal with the reality of what is all too frequently a series of destructive events that involve by their very nature the disintegration of the dying person's humanity. I have not often seen much dignity in the process by which we die.
Several years ago, I realized that I didn’t want to spend all my life in medicine. It had me in a sort of spiritual box, like a plant whose roots are getting crowded. I felt I wasn’t growing. So I promised myself that I would quit while I still had the energy to get involved in something new. There’s nothing wrong with medicine. There’s more paperwork, more lawsuits, less understanding between doctors and patients. But it’s still a great business. But not for me - not any longer.
What is troubling is not just being average but settling for it. Everyone knows that average-ness is, for most of us, our fate. And in certain matters—looks, money, tennis—we would do well to accept this. But in your surgeon, your child's pediatrician, your police department, your local high school? When the stakes are our lives and the lives of our children, we want no one to settle for average.
[We think our job is to ensure health and survival. But really it is larger than that. It is to enable well-being. And well-being is about the reasons one wishes to be alive. Those reasons matter not just at the end of life, or when debility comes, but all along the way. Whenever serious sickness or injury strikes and your body or mind breaks down, the vital questions are the same: What is your understanding of the situation and its potential outcomes? What are your fears and what are your hopes? What are the trade-offs you are willing to make and not willing to make? And what is the course of action that best serves this understanding?
At least two kinds of courage are required in aging and sickness. The first is the courage to confront the reality of mortality- the courage to seek out the truth of what is to be feared and what is to be hoped. But even more daunting is the second kind of courage - the courage to act on the truth we find.
I always feel that young doctors are only too anxious too experiment. After they've whipped out all our teeth, and administered quantities of very peculiar glands, and removed bits of our insides, they then confess that nothing can be done for us. I really prefer the old-fashioned remedy of big black bottles of medicine. After all, one can always pour those down the sink.

Once home, it's difficult to adjust. Moreover, we often earn more than our husbands. It takes a generous and exceptional man to forgive all that.
The case which I reported on September 26, 1901, was really the last which occurred in Havana. Of course we did not know it at the time, but this case marked the first conquest of yellow fever in an endemic center; the first application of the mosquito theory to practical sanitary work in any disease.
Turning at the sound of voices, Amelia saw Merripen carrying her sister outside. Win was dressed in a nightgown and robe and swathed in a shawl, her slim arms looped around Merripen’s neck. With her white garments and blond hair and fair skin, Win was nearly colorless except for the flags of soft pink across her cheekbones and the vivid blue of her eyes. “… that was the most terrible medicine,” she was saying cheerfully. “It worked,” Merripen pointed out, bending to settle her carefully on the chaise. “That doesn’t mean I forgive you for bullying me into taking it.” “It was for your own good.” “You’re a bully,” Win repeated, smiling into his dark face. “Yes, I know,” Merripen murmured, tucking the lap blankets around her with extreme care. Delighted by the improvement in her sister’s condition, Amelia smiled. “He really is dreadful. But if he manages to persuade more villagers to help clean the house, you will have to forgive him, Win.” Win’s blue eyes twinkled. She spoke to Amelia, while her gaze remained on Merripen. “I have every faith in his powers of persuasion.
No matter who you were in sixteenth-century Europe, you could be sure of two things: you would be lucky to reach fifty years of age, and you could expect a life of discomfort and pain. Old age tires the body by thirty-five, Erasmus lamented, but half the population did not live beyond the age of twenty. There were doctors and there was medicine, but there does not seem to have been a great deal of healing. Anyone who could afford to seek a doctor's aid did so eagerly, but the doctor was as likely to maim or kill as to cure. His potions were usually noxious and sometimes fatal—but they could not have been as terrible and traumatic as the contemporary surgical methods. The surgeon and the Inquisitor differed only in their motivation: otherwise, their batteries of knives, saws, and tongs for slicing, piercing, burning, and amputating were barely distinguishable. Without any anesthetic other than strong liquor, an operation was as bad as the torments of hell.
I wonder if much that ails our society stems from the fact that we have allowed ourselves to be cut off from that love of, and from, the land. It is medicine for broken land and empty hearts.
The nineteenth century is called the century of hygiene, and the twentieth is the century of medicine. The twenty-first century may be the century of behavioral change. Changing everyday, long-term behaviors--how we exercise, what we eat--is the key to adding years and quality to our lives.
There is a saying about surgeons, meant as a reproof: "Sometimes wrong; never in doubt." But this seemed to me their strength. Each day surgeons are faced with uncertainties. Information is inadequate; the science is ambiguous; one's knowledge and abilities are never perfect. Even with the simplest operation, it cannot be taken for granted that a patient will come through better off - or even alive. Standing at the table my first time, I wondered how the surgeon knew that he would do this patient good, that all the steps would go as planned, that the bleeding would be controlled and infection would not take hold and organs would not be injured. He didn't, of course. But still he cut.
In these days before antiseptics, doctors themselves also suffered high mortality rates. Florence Nightingale, a nurse during the Crimean War (1853-1856), watched one particularly inept surgeon cut both himself and, somehow, a bystander while blundering about during an amputation. Both men contracted an infection and died, as did the patient. Nightingale commented that it was the only surgery she'd ever seen with 300 percent mortality.
The core predicament of medicine - the thing that makes being a patient so wrenching, being a doctor so difficult, and being a part of society that pays the bills they run up so vexing - is uncertainty. With all that we know nowadays about people and diseases and how to diagnose and treat them, it can be hard to see this, hard to grasp how deeply uncertainty runs. As a doctor, you come to find, however, that the struggle in caring for people is more often with what you do not know than what you do. Medicine's ground state is uncertainty. And wisdom - for both the patients and doctors - is defined by how one copes with it.
The simple view is that medicine exists to fight death and disease, and that is, of course, its most basic task. Death is the enemy. But the enemy has superior forces. Eventually, it wins. And, in a war that you cannot win, you don't want a general who fights to the point of total annihilation. You don't want Custer. You want Robert E. Lee, someone who knew how to fight for territory when he could and how to surrender when he couldn't, someone who understood that the damage is greatest if all you do is fight to the bitter end.
Our health care approaches squander billions on extravagant treatment regimes that end up accomplishing little, as a society we refuse to adopt the small, even tiny adjustments that could easily reduce the clawing uncertainties that now degrade millions.
They found the Infirmary of Justice much as they had left it: white institutional walls, too-bright floors, and a reassuring smell of antiseptic. Reassuring at least to Tara, because the smell signalled that the people running this infirmary knew about antiseptic.
The evening was a string of miserable minutes strung together in tiny clusters. Three minutes for a man shot through the shoulder; Ellis put first a finger in the entry wound and then another in the exit and when his fingers touched, he decided the man was only lightly injured and didn’t need a surgeon. Three minutes to set a broken wrist and splint it with a strip of cowhide and a piece of wood from a sycamore tree. Two minutes to tourniquet a leg, then extract a piece of wire deep in the meat of it. A minute to peek under a pink, saturated bandage several inches below a slender belly button; he saw thin, red water leaking from a hole and smelled urine, knew the ball had breached the bladder. It would either heal or it wouldn’t, but nothing to do about it so he set the soul aside, a case not to be operated upon. He turned a man’s head looking for the source of a trickle of blood and had ten terrible minutes trying to stop torrential bleeding from under his clavicle; frantic moments during which he could get neither a finger nor a clamp around the pulsating source. All bleeding stops eventually though, and the case did not violate the rule. State sponsored medicine and science can function as ideology, inspiring blind commitment, fanatical defensiveness and denial, particularly of outcomes inconsistent with the preferred explanatory model. The social etiology of compromised health, insists on an understanding of these conditions and the way they impact the objectivity or neutrality of scientific and medical interpretation.

In the spring of 2009, I was the 217th person ever to be diagnosed with anti-NMDA-receptor autoimmune encephalitis. Just a year later, that figure had doubled. Now the number is in the thousands. Yet Dr. Bailey, considered one of the best neurologists in the country, had never heard of it. When we live in a time when the rate of misdiagnoses has shown no improvement since the 1930s, the lesson here is that it’s important to always get a second opinion.While he may be an excellent doctor in many respects, Dr. Bailey is also, in some ways, a perfect example of what is wrong with medicine. I was just a number to him (and if he saw thirty-five patients a day, as he told me, that means I was one of a very large number). Ninety percent of the time, you can arrive at an uncannily accurate diagnosis by paying close attention, using physical examination and sophisticated lab test to confirm your hunch (and to increase the bill to the insurance company).
Doctors tend to enter the arenas of their profession's practice with a brisk good cheer that they have to then stop and try to mute a bit when the arena they're entering is a hospital's fifth floor, a psych ward, where brisk good cheer would amount to a kind of gloating. This is why doctors on psych wards so often wear a vaguely fake frown of puzzled concentration, if and when you see them in fifth-floor halls. And this is why a hospital M.D.--who's usually hale and pink-cheeked and poreless, and who almost always smells unusually clean and good--approaches any psych patient under this care with a professional manner somewhere between bland and deep, a distant but sincere concern that's divided evenly between the patient's subjective discomfort and the hard facts of the case.

When a theological world view dominated, deviance was sin; when the nation-states emerged from the decay of feudalism, most deviance became designated as crime; and in our own scientifically oriented world, various forms of deviance are designated increasingly as medical problems. Thus we view the medical paradigm as the ascending paradigm for deviance designations in our postindustrial society.

But the fact is, death is not a confrontation. It is simply an event in the sequence of nature's ongoing rhythms. Not death but disease is the real enemy, disease the malign force that requires confrontation. Death is the surcease that comes when the exhausting battle has been lost. Even the confrontation with disease should be approached with the realization that many of the sicknesses of our species are simply conveyances for the inexorable journey by which each of us is returned to the same state of physical, and perhaps spiritual, nonexistence from which we emerged at conception. Every triumph over some major pathology, no matter how ringing the victory, is only a reprieve from the inevitable end.

Perhaps the greatest danger in the way that alternative therapists behave is simply the promotion of their own treatments when patients should be in the care of a conventional doctor. There are numerous reports of patients with serious conditions (e.g. diabetes, cancer, AIDS) suffering harm after following irresponsible advice form alternative practitioners instead of following the advice of a doctor.
(Florence) Nightingale's passion for statistics enabled her to persuade the government of the importance of a whole series of health reforms. for example, many people had argued that training nurses was a waste of time, because patients cared for by trained nurses actually had a higher mortality rate than those treated by untrained staff. Nightingale, however, pointed out that this was only because more serious cases were being sent to those wards with trained nurses. If the intention is to compare the results from two groups, then it is essential to assign patients randomly to the two groups. Sure enough, when Nightingale set up trials in which patients were randomly assigned to trained and untrained nurses, it became clear that the cohort of patients treated by trained nurses fared much better than their counterparts in wards with untrained nurses.

For all its subtleties and infinite beauty, life has but one purpose: to keep the wheel turning.
I know you're on my side," an immunologist once remarked to me as we discussed the politics of vaccination. I did not agree with him, but only because I was uncomfortable with both sides, as I had seen them delineated. The debate over vaccination tends to be described with what the philosopher of science Donna Haraway would call "troubling dualisms." These dualisms pit science against nature, public against private, truth against imagination, self against other, thought against emotion, and man against woman.
A slow but steady transformation of deviance has taken place in American society. It has not been a change in behavior as such, but in how behavior is defined. Deviant behaviors that were once defined as immoral, sinful, or criminal have been given medical meanings. Some say that rehabilitation has replaced punishment, but in many cases medical treatments have become a new form of punishment and social control.
And when comfort is what we want, one of the most powerful tonics alternative medicine offers is the word 'natural.' This word implies a medicine untroubled by human limitations, contrived wholly by nature or God or perhaps intelligent design. What 'natural' has come to mean to us in the context of medicine is 'pure' and 'safe' and 'benign'. But the use of 'natural' as a synonym for 'good' is almost certainly a product of our profound alienation from the natural world.
In a sense, the Earth is mounting an immune response against the human species. It is beginning to react to the human parasite, the flooding infection of people, the dead spots of the concrete all over the planet, the cancerous rot-outs in Europe, Japan and the United States, thick with replicating primates, the colonies enlarging and spreading and threatening to shock the biosphere with mass extinctions. Perhaps the biosphere does not 'like' the idea of five billion humans.
I would not be among you to-night (being awarded the 1964 Nobel Prize in Physiology or Medicine) but for the mentors, colleagues and students who have guided and aided me throughout my scientific life. I wish I could name them all and tell you their contributions. More, however, than anyone else it was the late Rudolf Schoenheimer, a brilliant scholar and a man of infectious enthusiasm, who introduced me to the wonders of Biochemistry. Ever since, I have been happy to have chosen science as my career, and, to borrow a phrase of Jacques Barzun, have felt that 'Science is, in the best and strictest sense, glorious entertainment'.
Where there were once several competing approaches to medicine, there is now only one that matters to most hospitals, insurers, and the vast majority of the public. One that has been shaped to a great degree by the successful development of potent cures that followed the discovery of sulfa drugs. They routinely perform what were once considered miracles. The result, in the view of some, has been a shift in the profession from caregiver to technician. The powerful new drugs changed how care was given as well as who gave it.

It has been demonstrated that a species of penicillium produces in culture a very powerful antibacterial substance which affects different bacteria in different degrees. Generally speaking it may be said that the least sensitive bacteria are the Gram-negative bacilli, and the most susceptible are the pyogenic cocci ... In addition to its possible use in the treatment of bacterial infections penicillin is certainly useful... for its power of inhibiting unwanted microbes in bacterial cultures so that penicillin insensitive bacteria can readily be isolated.
Physicians, patients, and ethicists must also understand that acknowledging abuse and encouraging African Americans to participate in research are compatible goals. History and today's deplorable African American health profile tell us clearly that black Americans need both more research and more vigilance.
..I don't count Jennifer among my mistakes. She had a severe infection and precious little reserve. Nevertheless, I think of her often. Those minutes of terror and confusion I felt standing powerless in her room served as a visceral reminder throughout my training... that the big picture isn't enough in medicine...
Those in the United States who by and large have the best medical care and advice readily available to them at the least expense are the families of the specialists in internal medicine. These families use less medicine and undergo less surgery on the whole than any other group rich or poor.
In my youth once when I had a really exquisite toothache I suddenly realized that my tooth had temporarily become the centre of the universe that its outcries were more important than anything else and that I would do absolutely anything to placate it. And as one gets older and starts worrying about cancer one becomes more and more conscious of the fragility of the whole body and with that consciousness comes a new and degrading kind of fear. It is degrading because it strengthens the desire to survive on any terms and the desire to survive on any terms is the most base of all our instincts.
Tranquillizers do not change our environment nor do they change our personalities. They merely reduce our responsiveness to stimuli. They dull the keen edge of the angers fears or anxiety with which we might otherwise react to the problems of living. Once the response has been dulled the irritating surface noise of living muted or eliminated the spark and brilliance are also gone.
